PLAYBOY SUBMISSION APPLICATION FORM

THIS IS AN APPLICATION FOR CONSIDERATION TO SUBMIT TO PLAYBOY ONLY!

If selected, your photos will be taken and submitted to the Playboy main magazine and Playboy Special Editions editor/editor's assistant for review and consideration.
You must be at least 18 years of age or older on the date of the shoot with 2 forms of ID to prove age (at least one must be a government issued ID with a photo).
Playboy's application/bio and a release will be provided and are required at the time of the shoot and the style of the shoot will be topless to conservative "Playboy" style nudes.
If you are approved, there is no charge for this service to keep these photos confidential.
There is no pay implied or suggested unless selected by Playboy and they will compensate you based upon their standard fees and compensation packages.

I have two options for this shoot, you will need to select the option that most fits your needs.

1. WITH MAKE-UP/HAIR-A make-up artist is available for hire at their current rate of $100-200 and this is your option and responsibility.
OR
2. WITHOUT MAKE-UP/HAIR-You elect to do your own make-up/hair and there is no fee required for the submissions.
